DRIVING PERSONAL VEHICLE FOR FLM EVENTS
Please note the following requirements for submitting this form:
Under the direction of our insurance company, we have implemented guidelines for people driving their personal vehicles for FLM events (for example, JAM teachers driving kids to the bowling ally, etc). In order to serve as a driver for an FLM event, the driver must meet the following criteria:
- Be 21 or older
- Have a valid driver’s license & auto insurance coverage
- No tickets or accidents in the past 3 years (an accident that you were ticketed or found at fault for)
Download the form that all drivers for events must fill out. The Dept Head must have the driver(s) complete this form prior to the event, and turn them into the church office to Angela King’s attention.
Drivers must complete this form each time they drive for an event.
