SECURE WORKER APPLICATION
Use this form is you would like to serve in:
KidzTown, Youth Ministries, Nursing Home, Prison Ministry, Inside/Outside Security, Special Detail, or Pastoral Care
MINOR WORKER APPLICATION
Use this form if the youth applicant is in 6th grade or above and would like to serve in:
*Non-Secure Helps Ministry Departments (6th grade or above) *Transformed Twos through Kingdom Kids: (6th Grade or above)
*Bible Babies, Wonderful Ones, or Uptown Kids: (9th Grade or above).
GENERAL WORKER APPLICATION
Use this form if would like to serve in a department not mentioned under the “Secure Worker Application”
